We constantly seek out and engage with partners who share our commitment to population health, creating innovative methods and models of care that provide the highest quality and best results at a fair cost to our patients. That commitment has resulted in a growing number of collaborations, nationally and regionally.

National collaborations

The High Value Healthcare Collaborative

In December 2010, we, along with Mayo Clinic, Denver Health, Intermountain Healthcare, and The Dartmouth Institute for Health Policy and Clinical Practice (TDI), announced the formation of the High Value Healthcare Collaborative (HVHC) to improve health care, lower costs, and move best practices out to the national provider community. The collaborative identified 9 high-volume, high-cost, high-variation conditions to focus on, with the goal of improving care and outcomes, reducing variation, and lowering costs. Since its founding, HVHC has expanded to nearly 20 health systems. And its first research, on total knee replacement, has been published.

OneCare Vermont

We have partnered with University of Vermont Medical Center to create OneCare Vermont in 2012. Today, OneCare is Vermont’s local, independent, 501(c)(3) accountable care organization (ACO), partnering with health care providers to transform the state’s health care system. We remain a member of OneCare because we believe in offering the best care possible to our patients, and because we support the shift to value-based care.

OneCare doesn’t provide health care—instead, it brings health groups together to improve health care quality and stabilize costs. By managing network performance, giving health care providers data and analytics, and paying for quality of care and improved population health, OneCare supports organizations like ours in our work to deliver excellent health care. Through this partnership, OneCare moves Vermont toward a trusted, equitable health care system where Vermonters and providers work together to achieve optimal health and an exceptional care experience.

Regional and state collaborations and affiliations

The Center for Rural Emergency Services and Trauma

The Center for Rural Emergency Services and Trauma (CREST) is a Dartmouth Hitchcock Medical Center-based collaborative program of 17 critical access hospitals and community hospitals. Founded at Dartmouth Hitchcock Medical Center in 2007, CREST is designed to improve the emergency care of patients in rural New England and is one of Connected Care's primary educational and outreach partners. CREST is located in Lebanon, New Hampshire, within our Emergency Department and the Trauma Center, a Level I, university-affiliated trauma center.

New England Alliance for Health

New England Alliance for Health (NEAH) is a group of community hospitals, behavioral health centers, and home healthcare agencies that came together in a shared commitment to improve the quality, efficiency, and availability of health care in New Hampshire, Vermont, and western Massachusetts. As a group, NEAH members share knowledge, resources, and solutions to challenging healthcare issues. In our local communities, we treat illness and improve wellness. We work with tax-exempt charitable health care organizations in thoughtful, patient-focused collaboration to improve patient care systems and the health status of all communities in the region.
